 AFFINITY HEALTH CARE LLC JOB DESCRIPTION FOR REHABILITATION ASSISTANT Must be 18 years of age or older High School Diploma or equivalent Free from communicable disease Effective oral and written communication skills Ability to perform basic math skills Have the ability to perform “major life activities” as described in the EECC laws, Chapter 29, CFR (These “major life activities” are those basic activities that the average person in the general population can perform with little or no difficulty, i.e., caring for oneself, performing casual walking, seeing, hearing, speaking, breathing, learning and working. Other activities, but not limited to, sitting, standing, lifting (50lbs.), reaching, etc. Must be able to complete successfully all trainings required by Affinity Health Care and the State/County. Implement each consumer’s Individual Service Plan (ISP) Assist, coach, teach, support and monitor consumer’s in their activities of daily living and psycho-social functioning. Complete medical record charting and other required documentation and paperwork Maintain confidentiality regarding the consumer’s. Establish appropriate working relationships with supervisors and peers Maintain the upkeep of the home. Transport and supervise consumers in the community, for both recreational and therapeutic activities.
